In preparation for your consultation appointment, if your glasses prescription is less than one year old, we will need to request eye exam records that are older than one year. In order to request a patient’s medical records we need to have the following form filled out. The form must be signed by either the patient or their legal representative. The form can be printed and submitted in office, or via fax: 719-258-1261.
